Evergreen Children's Clinic is located in Puyallup, Washington not far from the Puyallup Fairgrounds. If you are traveling in either direction on Highway 512 get off at the Fairground Exit. Continue south (up the hill) on Meridian towards South Hill. The clinic is located at 19th and Meridian Street South, on the right (west) side of the street. You'll recognize you've arrived when you see our sign.

Our clinic is an inviting modern facility designed especially for providing care for kids. It is completely handicapped accessible.

Office Hours and Appointments
Evergreen Children's Clinic is open Monday through Friday from 8:00 am to 5:00 pm and on Saturdays from 8:30 am - 12:00 pm. There is always a Pediatrician available for emergency calls, 24 hours a day, 7 days per week.

We have both a same day "open access" appointment system and a future appointment schedule. To make an appointment, call 253-848-2303. Please remember to notify us at least 24 hours prior to your appointment if you are unable to make your scheduled time. We will attempt to re-schedule you at a more convenient time. Missed appointments without prior notification may result in dismissal from the practice.

At the time of your first appointment, you will be asked to review the HIPPA required Notice of Privacy Practices Statement. Pediatrics PM
If your child needs to be seen after hours, visit our Pediatrics PM clinic. Pediatrics PM provides walk-in pediatric care for acute illnesses and minor injuries. Pediatrics PM is open to the entire community.

After Hours Care
Recognizing that potentially urgent pediatric problems do not always occur during office hours, we have 24-hour physician coverage available. By calling our office number, 253-848-2303, you will access our answering service. The service will then contact our on-call physician.

After hours pediatric care is also available for sick kids through Pediatrics PM. Pediatrics PM is located on the lower level of the Evergreen Children's Clinic.

Prescriptions
Our policy is very firm on not "calling in" prescription medications for children. We cannot intelligently treat a child we have not had the opportunity to evaluate. Medication refills are handled during office hours only, so please try to anticipate your medication needs before evening hours, weekends or holidays.

Insurance Billing
We will be happy to bill your insurance company for you. In order to do this, we need to have the assignment of benefits portion of your registration form signed and a current insurance card on file in our office.

If we are doing your insurance billing, we will require the deductible or co-payment portion of your bill to be paid at the time of the office visit. If you have elected to do your own insurance billing, we will ask for the entire payment to be made at the time of service.

If you are having any trouble making payments on your bill, please let us know of your problem and we will endeavor to make your payments as convenient as possible. Remember, we only know there is a problem if you tell us. You can reach us by calling 253-848-2303.

It is also important to note that the insurance companies require us to treat a medical problem cared for at a "'Preventative Medicine Visit" (Well-Child or Physical) as an "Independent Visit", with a separate co-pay.

We have the capability of billing your secondary insurance for you. You will need to provide a current secondary insurance card for our office to keep on file.

Unfortunately, we cannot bill "third-party" insurances, such as auto or homeowners insurance. We ask that you pay for the visit at the time of service and request reimbursement from these insurance companies yourself.

Special Insurance Plans
There has been a great proliferation of Preferred Provider and Managed Care Plans in the last several years. Many of these plans have special criteria for hospitalizations and/or consultative care. If these specific criteria are not followed precisely, the insurance company may not pay part, or all of your child's medical costs.

We participate in many commercial and managed care plans, including state sponsored plans.

We are understanding of the additional burden this puts on families at a time of stress. However, to avoid future problems, it is essential that we know this information prior to any hospitalization or consultative referral.
